The Adoption Dossier shall be submitted with your initial application to the Minister of Justice. It shall include:
1. Application to the Minister of Justice of the
a. personal data (full name, DOB and place of birth), nationality and citizenship, No., date and place of issuance of the ID document, country habitual residence, permanent and current address;
b. short history of your family;
c. information about your economical and social status;
d. information about Dreams Foundation – persons for contact, address, phone, license, representation;
e. signatures.
2. Permission for adoption issued by BCIS – approval of your I-600A (I-171H);
3. Certificate issued by the competent authority that you are not deprived of parental rights;
4. Home Study prepared by licensed private or authorized government agency at the place of your residence, containing: information about your personal identity; your ability and suitability to adopt; your detailed characteristic – personal, family, health and social status; your social environment and living conditions; your motivation for adoption; the characteristics of the children of whom you would be appropriate to take care, as well as any other information required by the law;
5. License of the agency that conducted the home study;
6. Medical Certificates for the health condition of each of the adoptive parents reflecting your physical and psychical condition and proving lack of serious chronic, contagious and venereal diseases, AIDS, tuberculosis, etc.
7. Certificate of criminal record – both local and FBI;
8. Marriage license/Document proving single status;
9. Power of attorney in favor of Dreams Foundation for representation in the adoption procedure;
10. Intermediation contract with Dreams Foundation;
11. Certified copies of your passports OR certified birth certificates;
12. Employment letter issued by your employers or accountants – in case you own a business – showing your income;
13. Photos of your family and house/apartment.
(The last two documents are not required by the law but we find it helpful if they are submitted too).
All documents should be originals, notarized and apostilled by the respective state.
